Liberation Programs' Families in Recovery (FIRP) is an inpatient residential program in Norwalk, CT for pregnant and parenting women with children. FIRP has been in existence since 1994 and is unique in our region. It is the only inpatient residential treatment program for pregnant and parenting women in Fairfield County. Mothers can have up to two children ages birth to 10 years old, and FIRP is the only inpatient program in Connecticut where mothers can bring two children up to the age of 10 into the program with them. FIRP is gender and trauma sensitive with an emphasis on individual and family needs. Throughout treatment, mothers are supported in managing challenges related to substance use disorder (SUD), building, enhancing and maintaining life skills, peer-to-peer support and accessing resources to provide safe, nurturing homes for their children and themselves as they prepare to transition back to the community.

Care Coordinator - Families in Recovery Program (FIRP)

The purpose of the Care Coordinator position is to help Recoverees maintain housing stability, sobriety, independent living skills and self-sufficiency. The Care Coordinator is expected to fulfill these expectations through the development and implementation of engagement strategies aimed at securing the residents' voluntary acceptance of a wide variety of supports and services.

Duties

Linkage to Care & Services Take steps necessary to assist recoverees in meeting basic needs and active planning for post-discharge. Services shall at minimum include addressing: medical services, behavioral healthcare, entitlement supports, vocational/educational services, clothing and food assistance, etc. In addition to linking the recoverees to community resources the Care Coordinator may also advocate, educate and provide other reasonable supports enabling recoverees to be successful in accessing care and services in the community,

Documentation - Clearly documents all encounters with recoverees as well as all services provided on recoverees behalf. Meet with recoverees regularly to better understand and document case management needs.

Qualifications

One year of experience in the provision of case management and care coordination services in the substance abuse or mental health field

Must have strong oral and written communications skills, time management, and interpersonal skills to work effectively at all levels of the organization and its stakeholders.
